new phone line's input fields are shown, but not usable for input until you hit "create a new phone line"

Web Admin -> Wizard -> Phone Lines

The input fields are not usable until you click "Add new phone line" These fields should either be hidden until you have clicked that, or it should be implied that when you hit save on an empty form, you want to create a new phone line.
